Transition Resources & Community Supports

Located in Braintree, Aspire Health Alliance’s Transition Resources and Community Supports (TRACS) program serves 16-to-25-year-olds who are currently in treatment for mental illness or co-occurring mental illness and substance abuse issues.

Focusing on family, relationships, education, employment, and other young-adult issues, TRACS connects clients with mentors and peers who have shared similar experiences and can provide first-hand advice on overcoming challenges, gaining independence, and achieving success.

The TRACS team also offers opportunities for clients to develop knowledge and friendships, express themselves creatively, and seek gainful employment through workshops, social events, Stepping Stones groups, and Photovoice. TRACS also has a monthly newsletter jammed packed with monthly activities, client artwork, all groups, movie night and much more. You can download the February 2020 Newsletter here.

Aspire Health Alliance’s TRACS program is available to young adults in Abington, Avon, Braintree, Bridgewater, Brockton, Carver, Cohasset, Duxbury, East Bridgewater, Easton, Halifax, Hanover, Hanson, Hingham, Hull, Holbrook, Kingston, Marshfield, Milton, Norwell, Pembroke, Plymouth, Plympton, Quincy, Randolph, Rockland, Scituate, Stoughton, West Bridgewater, Weymouth, and Whitman.

Photovoice

Photovoice, a project of our Transition Resources and Community Supports program, equips TRACS clients with cameras so that they can creatively document their journeys of healing and self-discovery.

A unique blend of therapy and photojournalism, the program enables participants to express their thoughts, feelings and perspectives through the art of photography.

Since the program’s inception, countless Photovoice stories and photos have proudly been displayed in many venues, including the Massachusetts State House.
