Transition Resources & Community Supports

Located in downtown Quincy, Aspire Health Alliance’s Transition Resources and Community Supports (TRACS) program serves 16-to-25-year-olds who are currently in treatment for mental illness or co-occurring mental illness and substance abuse issues.

Focusing on family, relationships, education, employment, and other young-adult issues, TRACS connects clients with mentors and peers who have shared similar experiences and can provide first-hand advice on overcoming challenges, gaining independence, and achieving success.

The TRACS team also offers opportunities for clients to develop knowledge and friendships, express themselves creatively, and seek gainful employment through workshops, social events, and our Photovoice, Stepping Stones, and Successful Employment Program (SEP).

Aspire Health Alliance’s TRACS program is available to young adults in Abington, Avon, Braintree, Bridgewater, Brockton, Carver, Cohasset, Duxbury, East Bridgewater, Easton, Halifax, Hanover, Hanson, Hingham, Hull, Holbrook, Kingston, Marshfield, Milton, Norwell, Pembroke, Plymouth, Plympton, Quincy, Randolph, Rockland, Scituate, Stoughton, West Bridgewater, Weymouth, and Whitman.

Successful Employment Program

Our Successful Employment Program (SEP) serves young adults who have been treated for a major mental illness within the past year and are searching for employment. Available to 16-to-25-year-olds, SEP is a voluntary program that seeks to facilitate client recovery by increasing accountability, responsibility and independence within the community.

Aspire Health Alliance’s SEP employment specialists work closely with clients to help them research job opportunities, complete and submit employment applications, attend job interviews, and provide on-site support and feedback once employed.

SEP also offers monthly vocational workshops and recreational activities, along with a weekly peer support group where clients can share experiences, participate in life skill exercises, and listen and learn about the latest employment topics.

Photovoice
Photovoice, a project of our Transition Resources and Community Supports program, equips TRACS clients with cameras so that they can creatively document their journeys of healing and self-discovery.

A unique blend of therapy and photojournalism, the program enables participants to express their thoughts, feelings and perspectives through the art of photography.

Since the program’s inception, countless Photovoice stories and photos have proudly been displayed in many venues, including the Massachusetts State House.
