Job Description

Under the direction of the Adult Community Clinical Service (ACCS) Director, responsibilities include ensuring that each client receives their prescribed medical regime and advocating and ensuring the health and safety of each individual served in the program. Communicates and records health related information, as well as relays this information to program staff through in-services, trainings and supervision. He/she will participate on the Recovery Planning Team, monitoring and evaluation of IAP service delivery. He/she will direct specialty services coordination as required by IAP’s. This position requires valid driver’s license and access to personal vehicle for client transportation.

 Assesses the individual health needs of each program participant and provides or advocates necessary services.

  • Is flexible and creative when making decisions on behalf of the participants.
  • Is knowledgeable of special needs of the participants.
  • Informs the MD of health care problems requiring treatment.
  • Assists in documenting acute illness, accidents and injuries.
  • Promptly communicates data about individual’s health problems to their residence.

Implements the prescribed medical regime within the program by collaborating and communicating with medical providers to ensure health services are provided.

  • Refers health care problems to the individual’s doctors.
  • Interacts with medical providers in a courteous and respectful manner.
  • Follows up on problems referred to medical providers to ensure the problems have been addressed.
  • Notes, transcribes and communicates MD orders with 100% accuracy.
  • Assists staff in presenting organized, valid and objective data to medical providers.
  • Documents any changes in the individual’s health regime.

Maintains 100% compliance with SSMH and Licensing Agencies policies for medication regimes.

  • Provides consultation to staff regarding the effectiveness of the individual’s medication regime.
  • Refers adverse side effects to the individual’s medical provider in a timely manner to ensure safety.
  • Provides direct nursing services to acutely ill and/or injured individuals by implementing first aid and/or emergency medical response procedures to meet the individual’s needs.
  • Maintains current CPR and first aid certification.
  • Performs first aid procedures with accuracy, safety, skill, efficiency and effectiveness.
  • Informs the residential staff of any individual’s health related services that need to be provided.
  • Educates staff in administering medications to individuals to ensure medical regimes are followed.

Evaluates and individual’s response to nursing intervention by using current data to determine if objectives are met.

  • Collects data from staff to evaluate the individual’s response to nursing intervention.
  • Documents referral of health care problems to other disciplines or agencies.

 Supports the growth and effectiveness of the ACCS Residential Program by providing consultation and training to staff.

  • Utilizes the ACCS Residential team members as a resource to address specific issues.
  • Assists with orientation of new staff to program areas and issues.
  • Acts as a resource to peers regarding the ACCS Residential program.

Makes decisions by following established Program, Agency and licensing agencies policies and procedures.

  • Communicates significant changes in an individual’s health status to supervisor and residential staff.
  • Communicates and service delivery problems to support services.
  • Consults with ACCS Director on issues regarding direct care staff.

Ensures all MAP regulations are being followed.

  • Performs regular MAP audits at all programs.
  • Follows up on all MOR’s to ensure proper client care.
  • Follows up with all staff to ensure they are properly trained and educated on MAP.

Required Skills

  • Excellent communication and organizational skills.
  • Knowledge and experience in recovery based services preferred.

Required Experience

  • Current valid MA Registered Nurse licensure.
  • Minimum of four years of experience working with individuals with mental illness.
  • Minimum of two years of experience in interdisciplinary collaboration.
