Aspire Health Alliance is partnering with South Shore Health’s new and innovative Mobile Integrated Health (MIH) program, bringing hospital care directly to patients in the community in a high-touch, high-tech intervention to help avoid unnecessary emergency room visits. South Shore Health’s Emergency Medical Services (EMS) team will implement the program using mobile resources to deliver services to adults with chronic and co-morbid conditions and Aspire will provide mental health crisis services when needed.
