3445Job Description
Under the supervision of the Adult Community Clinical Services (ACCS) Assistant Director provides clinical assessment, risk/safety planning and treatment planning for persons in ACCS services. Leads development of comprehensive recovery based services to adults with serious mental illness. Coordinates multi-disciplinary team and coordinates/evaluates program services.
- Provides clinical leadership for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
- Assumes primary responsibility for developing, implementing, evaluating and revising overall treatment goals and plans in conjunction with the team
- Conducts comprehensive assessment of psychiatric history and comprehensive assessment and safety planning for all risk issues
- Provides on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff
- Documents client progress to maintain a permanent record of client activity according to established methods and procedures
- Participates in providing substance use and treatment services as needed
- Communicates with Guardians, Family members, doctors, hospitals, therapists, Human Rights Coordinator and DMH regularly to coordinate schedules, supports and services
- Reviews implementation and progress of IAP interventions
- Leads in all client intake and assessment processes
- Explores social & support opportunities for individuals to participate in the community
Required Skills
- Demonstrated ability to work successfully at coordinating care across a continuum of care required
- Must demonstrate the following characteristics: Flexibility with schedule, ability to function as a team member in assessing the needs of individuals, the ability to contribute in the development of policies and procedures, creativity and resourcefulness in seeking pathways that will provide support to the program
- Bilingual skills and/or experience with multicultural populations preferred
Required Experience
- Master’s Degree in Psychology, Social Work or related field
- License Required
- Must have at least 2 years’ experience working with individuals with SPMI
- Familiarity with principles of psychiatric recovery required
